By acknowledging below, each player (or parent/guardian for players under 18) confirms that they are voluntarily participating in the Shada 3-on-3 Basketball Tournament. Each participant understands that basketball involves inherent risks of physical injury and agrees to release and hold harmless the tournament organizers, sponsors, hosts, volunteers, the school district, and the City of Pelican Rapids from any liability arising from their participation. Each participant confirms they are physically able to participate and acknowledges responsibility for their own medical insurance.
Players under 18 require a parent or guardian to acknowledge the waiver on their behalf.

Send your registration fee via Venmo or PayPal. Include your team name and confirmation number in the payment note.
